The Apple Watch launched as a lifestyle accessory, framed as "Apple's most personal device ever." Within just a few years, Apple felt the shifting winds and repositioned it as a medical device, so decisively that Tim Cook declared Apple's "greatest contribution to mankind" would ultimately be in health.

Written for Jan Pietzsch's course, Technology Assessment and Regulation of Medical Devices (BIOE 256 / MS&E 256), this paper traces that transformation: the rapid FDA De Novo clearances for the ECG App and Irregular Rhythm Notification Feature, the subsequent Sleep Apnea Notification, and the commercialization playbook (coding, reimbursement, and human factors validation) Apple used to bring each to market. It also models the health economics, projected cost savings in stroke and cardiovascular care, and cost-effectiveness well within standard thresholds, alongside the mounting competitive and legal pressures Apple faces from third-party developers.
